Social Worker (MSW) – CMC

This is a Contract position in Lubbock, TX posted March 26, 2020.

Job Summary: Provides education and support services to patients, families and staff.

Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families.

As a complex Discharge Planner and a member of the Case Management Department, works collaboratively with the interdisciplinary team to affect positive patient and family outcomes.

Serves as resource for less-experienced social workers.

Essential Functions Maintain a safe exercise environment.

Develop safe and effective choreography according to type of class and experience of class.

Demonstrates knowledge and ability to modify member care needs according to patient age.

Provide a safe warm-up and cool-down consisting of light aerobics and stretching for 5-10 minutes.

Monitor perceived exertion and heart rate during cardiovascular workout.

Select appropriate background music according to type of class and participant preference.

Ability to lead and motivate class participation Ability to substitute for water and land aerobics Assist in training Level 1 aerobic instructors in certification requirements.

Upon request or referral from any member of the healthcare team (i.e.

· physician, RN) and/or patient/family, will complete a comprehensive
· psychosocial assessment to evaluate patient/family care needs; will provide
· counseling/crisis intervention/support/education to patient/family regarding
· coping with hospitalization, disability, and chronic/terminal illness and initiate
· referrals to appropriate resources to meet the needs of the patient/family to
· ensure psychosocial and emotional support are addressed.

Serves as a resource to all members of the healthcare team regarding
· emotional, social, and psychosocial components of patient illnesses and will
· ensure understanding and integration of all aspects of the plan of care.

Adheres to the regulatory requirements for social work intervention/oversight
· based on state and federal guidelines (i.e.: ESRD, transplant and
· rehabilitation patients).

Maintains current knowledge and researches availability of community
· agencies/resources for social, emotional or financial assistance.

· Understands placement intricacies and can interpret requirements from
· state, local, and federal agencies to optimize placement of patients in
· the most appropriate setting based on patient’s needs.

In collaboration with the RN Case Manager, provides information, education
· to patient/family on community resources and options for post-acute care
· appropriate to the age of the patient served and will Initiate referrals to
· community agencies/resources as needed.

Facilitates discharge arrangements in collaboration with patient/family and
· the healthcare team ensuring arrangements meet patient’s continuing care
· needs.

Ensures services are arranged and communicated.

As needed,
· follows up internally to ensure services are provided as arranged.

· and records clinical, psychosocial and financial barriers to a
· smooth transition across the healthcare continuum and assists in identifying
· and facilitating system improvements.

Utilizes communication, negotiation, and advocacy skills with patient/family,
· staff, healthcare team, and community resources.

Will maintain collaborative
· relationship with post-acute providers and community services.

Serves as a consultant for processing issues such as guardianship,
· abuse/neglect, power of attorney, healthcare surrogate, advanced directives,
· and psychiatric involuntary admission.

· Initiates/completes forms required for post-acute placement.

As patient is transferred, communicates pertinent information to next Social
· Worker and Care Manager to transition the patient to the next level of care.

· Documents activities with entries that are consistent with meeting
· department/facility requirements, licensure and regulatory guidelines.

Supports interdisciplinary team conferences and contribute to the planning
· and review of care for patients presented as needed.

Adheres to standards, policies and procedures at both the Hospital and
· departmental level.

· Promotes a service orientation in the performance of position duties and
· responsibilities and in interactions with patients, hospital staff and visitors.

Minimum Position Qualifications: Education: Masters degree in Social Work Experience: Working knowledge/experience in managed care and payor issues License/Certification: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker in state of Texas (LCSW).

Allowed to work with temporary licensure, but must obtain full licensure within 6 months of employment.
